-Facilitating physician consults and patient transfers from facility to facility to ensure appropriate patient and system flow across the SHA.

-Collaborating with other stakeholders to complete repatriation of patients back to the appropriate home hospital.

-Coordinating the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the Screening tool for Continuing Care eligibility and placement and alternate level of care services placement within the SHA.

-Managing the placement of Continuing Care clients into the appropriate vacancy.

-Providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Directors of assigned Integrated Service Areas.

-Maintaining and Reviewing the Acute Care Dashboard for Bed management purposes.

Job Qualifications Experience Required Qualifications: Baccalaureate degree or Diploma in Nursing.

Knowledge of leadership theories and strategies in order to lead, encourage, advise and cooperate with colleagues to promote and maintain healthy, positive relationships while achieving client/patient, team and organizational goals within available resources.

Knowledge of current trends, issues and perspectives in healthcare.

Knowledge of communication techniques and approaches in order to adapt to multiple and ever-changing situations and environments.

Knowledge of critical thinking processes in order to creatively solve problems in an open and resourceful manner.

Knowledge of change theories in order to identify, promote and support change efforts.

Knowledge of group processes, dynamics and organizational culture and how they contribute to the functioning of an organization.

Current licensure with the SRNA.

Current CPR certification in Basic Life support at Level C.

Equivalent of 5 years recent experience in a medical, surgical, critical care or emergency department in an acute care facility is preferred.

Previous coordinator experience is an asset.

Basic computer skills.

Knowledge, Skills & Abilities: Demonstrated leadership with good organizational skills.

Above average communication skills.

Well developed analytical, judgement and decision-making skills, using a case management model approach.

Demonstrated clinical competence.

Ability to promote effective relationships with staff, supervisors, patients/clients, families, and physicians.

Familiarity with the acute care system of admission/discharge/transfer of patients.

Understand community-based linkages with admission/discharge/transfer of patients & clients.

Ability to manage Patient Flow using computerized applications and equipment.

Additional Information 94.24 hours of work in a 4 week rotation.
