We are seeking a relationship-oriented and results-driven Executive Director, Primary Care & Chronic Disease Management, who is committed to promoting the integration of community and primary care to support a community-based system for health.

Reporting directly to the Vice President, Regional Care Integration, the Executive Director will be a clear, caring, and courageous leader who is accountable for the overall leadership of Primary Care & Chronic Disease Management programs and strategies.

As part of the senior management team, the successful candidate will bring an evidence-based approach to healthcare, promote cultural safety, encourage new health care initiatives and programs, and champion quality and service in public health.

The Executive Director is accountable for: Leading the strategic planning, assessment and quality improvements in Primary Care andChronic Disease Management services, including the implementation of strategies to achieve Ministry of Health initiatives and directives; Leadingregional components of Fraser Health’s development of specialized community services for medically complex patients within an integrated primary and community care model region-wide; Working in partnership with the BC Cancer Agency to provide executive leadership to facilitate the development and implementation of an overall cancer care strategy; Leading the development and implementation of Stroke Services within Fraser Health, in collaboration with other Health Authorities and the Ministry of Health; Developing and facilitating effective partnerships with local divisions of family practice to create proactive change, in collaboration with local hospital and community leadership Overseeing the clinical operations of Community Respiratory Services; Representing Fraser Health as a key strategic leader for Primary Care andChronic Disease Managementto key stakeholders; Establishing linkages with primary care providers, including Family Physicians, Specialists and Nurse Practitioners, in order to collaborate on services that will further the Primary Care agenda; Leading the Practice Support Program across Fraser Health to optimize physician practices and enhance physician skills/knowledge to develop and expand primary care capacity.

T he successful candidate will have: Master’s Degree in Health, Business Administration or a related field; Senior leadership experience in a complex health care organization, or an equivalent combination of education, training and experience.

Detailed Overview Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families: Reporting to the Vice President, Regional Care Integration, the Executive Director is accountable for the overall leadership of Primary Care &Chronic Disease Management programs and strategies.

This position leads regional components of Fraser Health’s development of specialized community services for medically complex andfrail adults withinan integrated primary and community care model across the region;ensures the identification, evaluation and analysis of strategic issues for clinical and service teams, and is informed by evidence-based information and decision-making from key clinical stakeholders to ensure integrated transitional patient care within a community-based system for health.

Develops and fosters collaborative partnerships with the Program Medical Directors, Primary Care; Regional Medical Directors, Medicine; Executive Directors for Health Services Areas / Programs; Divisions of Family Practice and other stakeholders at local, regional, and provincial levels; develops strategies; plans and oversees the implementation of a primary care, chronic disease management framework to operationalize services for identified high risk adult patient populations such as stroke, oncology, respiratory, and cancer services.

Supports physician practice optimization by leading the Practice Support Program for Fraser Health.

Works collaboratively in a complex environment with key internal and external stakeholders, and promotes the integration of community and primary care to support a community-based system for health.

Establishes relationships with internal and external stakeholders in areas related to shared care, primary care, chronic disease management.

Responsibilities Accountable to lead the strategic planning, assessment and quality improvements in Primary Care andChronic Disease Management services across Fraser Health, including the implementation of strategies to achieve Ministry of Health initiatives and directives.

Leadsregional components of Fraser Health’s development of specialized community services for medically complex and frail adults within an integrated primary and community care model region-wide.

In partnership with the BC Cancer Agency, provides executive leadership to facilitate the development and implementation of an overall cancer care strategy across Fraser Health.

Leads the development and implementation of Stroke Services within Fraser Health, in collaboration with other Health Authorities and the Ministry of Health.

In collaboration with local hospital and community leadership, develops and facilitates effective partnerships with local divisions of family practice to create proactive change which achieves the vision of the Community Based System for Health.

Oversees the clinical operations of Community Respiratory Services including the implementation and operation of the BreatheWELL at Home program.

Represents Fraser Health as a key strategic leader for Primary Care and Chronic Disease Managementtoother Health Authorities, Ministry of Health, General Practice Services Committee (GPSC), and other key stakeholders both internal and external.

Collaborates with key leaders to ensure alignment of strategies and services to achieve Fraser Health’s operational and strategic goals.

Establishes linkages with primary care providers, including Family Physicians, Specialists and Nurse Practitioners, in order to collaborate on services that will further the Primary Care agenda.

Leads the Practice Support Program across Fraser Health to optimize physician practices and enhance physician skills/knowledge to develop and expand primary care capacity.

Oversees the development of interdisciplinary care teams.

Oversees and/or leads the creation and development of business cases to support new initiatives, innovated services and operational models within Primary Care &Chronic Disease Management.

Provides leadership and direction to designated team members and direct reports utilizing effective management techniques (ie coaching, mentoring, skill and leadership development, performance management, recruitment, compensation management).

Qualifications Education and Experience Master’s degree in a health related discipline, supplemented withten (10) to fifteen (15) years’ progressively senior management experience including leadership in a large health care organization; or an equivalent combination of education, training and experience.
